Position Description

Pride Health is seeking a RN Home Health for a 13-week contract (with high possibility of extension) at a healthcare facility in Tacoma, WA. This is a great opportunity for experienced candidates to work in a fast-paced healthcare environment while gaining valuable experience.

Position Details:

  • Job title: RN Home Health
  • Location: Tacoma, WA
  • Start Date: ASAP
  • Contract Length: 13 weeks
  • Shift: Days | 5X8s | 9AM - 5PM
  • Hours per Week: 40 hours
  • Travel Pay: $2,500/week

Key Responsibilities:

  • Conduct comprehensive assessments of home health and hospice patients to identify physical, psychosocial, and environmental needs, and develop individualized Plans of Care accordingly.
  • Deliver direct nursing care, including infection control and pain management, while documenting all interventions and patient progress to ensure continuity and quality of care.
  • Educate patients and families on care routines, medications, diet, and home safety, tailoring teaching methods to individual learning needs and cultural backgrounds.
  • Supervise and guide HHAs, STNAs, and LVNs to maintain high standards of service delivery and coordinate with physicians and interdisciplinary team members.
  • Manage referrals, comply with third-party payer requirements, participate in on-call rotations, and attend care planning and team meetings as needed.

Experience:

  • 2 years experience as an RN in a clinical setting, preferably in a home health or hospice setting
  • Homecare Homebase Software Experience
  • OASIS Experience

Certifications & Skills:

  • Active RN License
  • Valid Driver's License
  • BLS (AHA)

*Offered pay rate will be based on education, experience, and healthcare credentials.
